I.R.S. now has death forms available.

Good news! Death forms are now available from the I.R.S.! Failure to fill out the death form with a number two pencil and return it in the enclosed envelope will result in the revocation of your death. Wait until you die, of course, but then send it here.

The really good news is that the questions are all multiple choice.
